Forms (Only for IGSES)
You can find the latest versions of the official forms here.
The forms must be submitted to the Student Affairs Division by 5 pm, 7 days (excluding
Saturdays, Sundays, and holidays) prior to the date of implementation.
1. Notification of Internship
Notification of Internship must be submitted when you will participate in an internship regardless of whether or not you earn credits.
※ Please be sure to submit it since the information on the notification will be utilized for the survey by Jasso and other organizations.
- The Career and Scholarship Support Division is the point of contact for the following programs.
These programs are implemented by the Career and scholarship in cooperation with public organizations. - 1) Internship programs provided through Kyushu Internship Promotion Council（Summer・Spring）
- 2) Internship programs publicly recruited by central ministries and public organizations
- 3) Internship programs provided through C-Engine（Mainly for graduate students in Science.）
- 4) Internship programs provided through the Japan Association of Cooperate Executives
- 5) Other internship programs
- The IGSES is the point of contact for the following programs.
- Programs for students in IGSES ※e.g. Toshiba Internship … Contact: Student Affairs Section
- Programs that apply individually
- Programs provided by companies and organizations.
- For individual applications, please complete the procedures on your own by following the guidelines provided by each host company and organization.
2. Notification of Travel Overseas
Notification of Travel Overseas must be submitted when you travel overseas. (including traveling to your home country).
※Please be sure to submit it since the information on the notification will be utilized for the survey by Jasso and other organizations.
３. Notification of Change in Contact Information【English】
Notification of Change in Contact Information must be submitted when your home address, phone number, or email address has changed.
International students must also submit a photocopy of the residence card with a new home address along with a notification when your home address has been changed.
